minidumps: gdb-compatible, software controlled core dumps

We wanted a mechanism to get a usable core dump, without spending the time or space needed for a full dump of dozens of GB.  Additionally, we want fine-grained control to make sure certain small regions of memory are not present in the dump, like secret keys. All of this was possible by using a signal handler to write a file that is in ELF core format, with a few hooks implemented by our application to let the minidump system know about running threads, etc.
